No Brasil&#44; poucos dados est&#227;o dispon&#237;veis sobre esse assunto&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Trata&#8208;se de estudo transversal realizado em um Hospital Universit&#225;rio do Sul do Brasil&#46; Foram convidados a participar pacientes subsequentes de todas as idades&#44; que frequentaram o Ambulat&#243;rio de Dermatologia&#44; no per&#237;odo de junho de 2021 a mar&#231;o de 2022&#46; O crit&#233;rio de inclus&#227;o foi diagn&#243;stico de DA de acordo com o UK Working Party&#46; O crit&#233;rio de exclus&#227;o foi nunca ter utilizado corticosteroide t&#243;pico &#40;CST&#41; anteriormente&#46; Este estudo foi aprovado pelo Comit&#234; de &#201;tica em Pesquisa do hospital &#40;n&#250;mero 2021&#8208;0091&#41;&#46; Termo de consentimento informado foi fornecido e assinado pelo paciente ou pelo cuidador&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">Todas as an&#225;lises foram conduzidas no R Studio &#40;vers&#227;o 2022&#46;2&#46;2&#46;485&#41;&#44; ambiente para computa&#231;&#227;o estat&#237;stica R &#40;vers&#227;o 4&#46;2&#46;0&#41;&#46; As vari&#225;veis quantitativas foram analisadas de acordo com a simetria da distribui&#231;&#227;o&#44; expressa em m&#233;dia e desvio padr&#227;o ou mediana e intervalo interquartil&#46; Nesse contexto&#44; foram utilizados o teste <span class="elsevierStyleItalic">t</span> de Student ou o teste de Mann&#8208;Whitney para amostras independentes&#46; Para analisar a correla&#231;&#227;o entre qualidade de vida e escores de corticofobia foram utilizados os m&#233;todos de Spearman e Kendall&#46; Todas as an&#225;lises foram bicaudais&#44; e o valor de alfa foi estabelecido em 0&#44;05&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">O tamanho da amostra foi calculado para associar a ades&#227;o ao tratamento ao grau de corticofobia &#40;utilizando o question&#225;rio TOPICOP &#8211; Topical Corticosteroid Phobia &#8211;&#41; usando o programa PSS Health vers&#227;o 0&#46;1&#46;5&#44;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> com base nos dados de Lee et al&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Foram estabelecidos o poder em 80&#37; e n&#237;vel de signific&#226;ncia em 5&#37;&#46; Utilizando o tamanho do efeito de 0&#44;382 encontrado por Lee et al&#46;&#44; estimou&#8208;se tamanho de amostra de 75 pacientes&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">O TOPICOP &#233; um question&#225;rio validado que avalia a corticofobia&#46;<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1&#44;4</span></a> &#201; composto por 12 quest&#245;es&#44; divididas em dois dom&#237;nios&#58; preocupa&#231;&#245;es e cren&#231;as&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> Todas as perguntas oferecem quatro respostas poss&#237;veis&#44; e os resultados s&#227;o apresentados em porcentagens &#40;escores mais altos indicam maior corticofobia&#41;&#46; Para pacientes &#60;<span class="elsevierStyleHsp" style=""></span>17 anos&#44; o cuidador principal respondeu ao question&#225;rio TOPICOP&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">Foi inclu&#237;do no estudo 75 pacientes&#44; e a m&#233;dia do escore TOPICOP&#40;t&#41; foi 41&#44;44 &#40;&#177; 21&#44;58&#41;&#46; A <a class="elsevierStyleCrossRef" href="#tbl0005">tabela 1</a> resume as caracter&#237;sticas dos pacientes&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">N&#227;o foi encontrada associa&#231;&#227;o entre corticofobia e taxas de ades&#227;o autorreferida ao tratamento &#40;dicotomizada em dois grupos&#58; &#8220;sempre adere completamente&#8221; e &#8220;nem sempre adere&#8221;&#59; p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;3797&#41;&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">Os cuidadores com n&#237;vel de escolaridade mais elevado apresentaram menos corticofobia &#91;TOPICOP&#40;t&#41;&#93;&#44; como mostrado na <a class="elsevierStyleCrossRef" href="#fig0005">figura 1</a> &#40;p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;005132&#41;&#44; Entretanto&#44; aproximadamente metade dos pacientes do presente estudo tinha menos de 17 anos&#44; o que pode explicar mais corticofobia&#44; e o tamanho da amostra em Stalder et al&#46; era pequeno &#40;n<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>48&#41;&#46; Al&#233;m disso&#44; o Brasil &#233; um pa&#237;s com diferen&#231;as sociais&#44; educacionais e culturais substanciais entre suas regi&#245;es e estados&#46; Escores TOPICOP&#40;t&#41; mais elevados entre os cuidadores do que entre os pacientes adultos indicam que muitas vezes os pais est&#227;o mais preocupados com a sa&#250;de de seus filhos do que com a sua pr&#243;pria&#46; Um estudo portugu&#234;s recente n&#227;o encontrou resultados semelhantes&#59; entretanto&#44; os dois estudos utilizaram pontos de corte diferentes para separar crian&#231;as&#47;adolescentes de adultos&#44;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> e as popula&#231;&#245;es avaliadas eram distintas &#40;pacientes da Europa Ocidental em compara&#231;&#227;o com pacientes de um pa&#237;s em desenvolvimento da Am&#233;rica do Sul&#41;&#46; Tamb&#233;m n&#227;o avaliaram ades&#227;o ou qualidade de vida&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">N&#227;o foi encontrada associa&#231;&#227;o entre corticofobia e ades&#227;o&#46; Vale ressaltar que a ades&#227;o foi avaliada retrospectivamente&#44; em virtude de limita&#231;&#245;es metodol&#243;gicas&#44; o que poderia resultar em vi&#233;s de recorda&#231;&#227;o&#44; e foi autorrelatada pelos pacientes&#44; o que poderia gerar vi&#233;s de resposta&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Associa&#231;&#227;o entre corticofobia e ades&#227;o ao tratamento foi descrita anteriormente&#44;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> mas ainda n&#227;o &#233; um consenso&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> Curiosamente&#44; um estudo de Mueller et al&#46; descobriu que a corticofobia medida pela escala visual anal&#243;gica &#40;EVA&#41; estava associada a pior ades&#227;o&#59; entretanto&#44; ao aplicar o question&#225;rio TOPICOP na mesma amostra isso n&#227;o ocorreu&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a> Ainda n&#227;o foi definido ponto de corte nos escores do TOPICOP que possa ser classificado como clinicamente relevante&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Frequentemente s&#227;o atendidos pacientes no Sul do Brasil que n&#227;o sabem que uma formula&#231;&#227;o espec&#237;fica cont&#233;m esteroides&#46; Portanto&#44; embora as taxas de corticofobia tenham sido compar&#225;veis &#224;quelas observadas em pa&#237;ses desenvolvidos&#44; &#233; poss&#237;vel que n&#227;o afetem no mesmo grau a ades&#227;o ao tratamento no Brasil por esse motivo&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">Diferentemente do presente estudo&#44; trabalhos anteriores descreveram associa&#231;&#227;o entre n&#237;vel educacional e corticofobia&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> No entanto&#44; existe a possibilidade de que caracter&#237;sticas culturais possam estar envolvidas&#44; uma vez que a maioria dos estudos foi realizada em pa&#237;ses ocidentais do Hemisf&#233;rio Norte&#46; Um estudo chin&#234;s n&#227;o encontrou nenhuma associa&#231;&#227;o&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">O presente estudo sugere que as pessoas que obt&#234;m informa&#231;&#245;es sobre CST de amigos e familiares podem ter menos corticofobia&#44; o que est&#225; de acordo com Song et al&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> Isso &#233; especialmente relevante no Brasil&#44; onde os CST s&#227;o vendidos livremente em farm&#225;cias&#44; sem necessidade de receita m&#233;dica&#46; Mais da metade dos pacientes do presente estudo compraram CST sem receita m&#233;dica no passado&#46; Apenas 65&#37; responderam que o dermatologista era uma de suas principais fontes de informa&#231;&#227;o&#46; Isso indica a necessidade de educar melhor os pacientes ao prescrever tratamento para DA&#46; Curiosamente&#44; um recente estudo portugu&#234;s encontrou associa&#231;&#227;o inversa entre educa&#231;&#227;o em sa&#250;de e a corticofobia&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Parece n&#227;o haver associa&#231;&#227;o entre corticofobia e ades&#227;o ao tratamento no Sul do Brasil&#46; Foram convidados a participar pacientes subsequentes de todas as idades&#44; que frequentaram o Ambulat&#243;rio de Dermatologia&#44; no per&#237;odo de junho de 2021 a mar&#231;o de 2022&#46; O crit&#233;rio de inclus&#227;o foi diagn&#243;stico de DA de acordo com o UK Working Party&#46; O crit&#233;rio de exclus&#227;o foi nunca ter utilizado corticosteroide t&#243;pico &#40;CST&#41; anteriormente&#46; Este estudo foi aprovado pelo Comit&#234; de &#201;tica em Pesquisa do hospital &#40;n&#250;mero 2021&#8208;0091&#41;&#46; Termo de consentimento informado foi fornecido e assinado pelo paciente ou pelo cuidador&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">Todas as an&#225;lises foram conduzidas no R Studio &#40;vers&#227;o 2022&#46;2&#46;2&#46;485&#41;&#44; Para analisar a correla&#231;&#227;o entre qualidade de vida e escores de corticofobia foram utilizados os m&#233;todos de Spearman e Kendall&#46; Todas as an&#225;lises foram bicaudais&#44; e o valor de alfa foi estabelecido em 0&#44;05&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">O tamanho da amostra foi calculado para associar a ades&#227;o ao tratamento ao grau de corticofobia &#40;utilizando o question&#225;rio TOPICOP &#8211; Topical Corticosteroid Phobia &#8211;&#41; usando o programa PSS Health vers&#227;o 0&#46;1&#46;5&#44;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> com base nos dados de Lee et al&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> Foram estabelecidos o poder em 80&#37; e n&#237;vel de signific&#226;ncia em 5&#37;&#46; Utilizando o tamanho do efeito de 0&#44;382 encontrado por Lee et al&#46;&#44; estimou&#8208;se tamanho de amostra de 75 pacientes&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">O TOPICOP &#233; um question&#225;rio validado que avalia a corticofobia&#46;<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">1&#44;4</span></a> &#201; composto por 12 quest&#245;es&#44; divididas em dois dom&#237;nios&#58; preocupa&#231;&#245;es e cren&#231;as&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> Todas as perguntas oferecem quatro respostas poss&#237;veis&#44; e n&#227;o houve associa&#231;&#227;o entre os adultos &#40;<a class="elsevierStyleCrossRef" href="#fig0005">fig&#46; 1</a>&#59; p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;7856&#41;&#46; Os cuidadores tiveram escores mais altos do que os pacientes adultos no TOPICOP&#40;b&#41; &#40;p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;02648&#59; <a class="elsevierStyleCrossRef" href="#fig0010">fig&#46; 2</a>&#41;&#46; Adultos com maior n&#237;vel de escolaridade apresentaram melhor ades&#227;o &#40;p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;024&#41;&#46; Os escores do &#205;ndice de Qualidade de Vida em Dermatologia &#40;DLQI&#44; do ingl&#234;s <span class="elsevierStyleItalic">Dermatology Life Quality Index</span>&#41; n&#227;o se correlacionaram com corticofobia&#59; no entanto&#44; escores mais altos no &#205;ndice de Qualidade de Vida em Dermatologia Infantil &#40;CDLQI&#44; do ingl&#234;s <span class="elsevierStyleItalic">Children&#39;s Dermatology Life Quality Index&#41;</span> correlacionaram&#8208;se com escores mais altos no TOPICOP&#40;t&#41; &#40;<a class="elsevierStyleCrossRef" href="#fig0015">fig&#46; 3</a>&#59; p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;0086&#41;&#46; A ferramenta SCORAD &#40;&#8220;SCORing Atopic Dermatitis&#8221;&#41; n&#227;o se correlacionou com o TOPICOP&#40;t&#41; &#40;p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;3189&#41;&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Foram avaliadas fontes de informa&#231;&#227;o sobre CST &#40;foi permitida mais de uma op&#231;&#227;o&#41; apresentaram menos corticofobia do que aqueles que n&#227;o o fizeram&#44; na an&#225;lise univariada &#40;p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;028&#41;&#46; Contudo&#44; o presente estudo n&#227;o teve poder suficiente para realizar an&#225;lise multivariada&#44; e essa informa&#231;&#227;o precisa ser confirmada com estudos adicionais&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">Os escores TOPICOP&#40;t&#41; entre os cuidadores foram semelhantes &#224;queles descritos anteriormente&#46;<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">5&#44;6</span></a> Os escores gerais do TOPICOP&#40;t&#41; foram mais altos do que aqueles relatados por Stalder et al&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> em S&#227;o Paulo&#44; Brasil &#40;m&#233;dia<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>33&#44;5&#41;&#46; Entretanto&#44; aproximadamente metade dos pacientes do presente estudo tinha menos de 17 anos&#44; o que pode explicar mais corticofobia&#44; e o tamanho da amostra em Stalder et al&#46; era pequeno &#40;n<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>48&#41;&#46; Al&#233;m disso&#44; o Brasil &#233; um pa&#237;s com diferen&#231;as sociais&#44; educacionais e culturais substanciais entre suas regi&#245;es e estados&#46; Escores TOPICOP&#40;t&#41; mais elevados entre os cuidadores do que entre os pacientes adultos indicam que muitas vezes os pais est&#227;o mais preocupados com a sa&#250;de de seus filhos do que com a sua pr&#243;pria&#46; Um estudo portugu&#234;s recente n&#227;o encontrou resultados semelhantes&#59; entretanto&#44; os dois estudos utilizaram pontos de corte diferentes para separar crian&#231;as&#47;adolescentes de adultos&#44;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a> e as popula&#231;&#245;es avaliadas eram distintas &#40;pacientes da Europa Ocidental em compara&#231;&#227;o com pacientes de um pa&#237;s em desenvolvimento da Am&#233;rica do Sul&#41;&#46; Tamb&#233;m n&#227;o avaliaram ades&#227;o ou qualidade de vida&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">N&#227;o foi encontrada associa&#231;&#227;o entre corticofobia e ades&#227;o&#46; Vale ressaltar que a ades&#227;o foi avaliada retrospectivamente&#44; em virtude de limita&#231;&#245;es metodol&#243;gicas&#44; o que poderia resultar em vi&#233;s de recorda&#231;&#227;o&#44; e foi autorrelatada pelos pacientes&#44; o que poderia gerar vi&#233;s de resposta&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">Associa&#231;&#227;o entre corticofobia e ades&#227;o ao tratamento foi descrita anteriormente&#44;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> mas ainda n&#227;o &#233; um consenso&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a> Curiosamente&#44; um estudo de Mueller et al&#46; descobriu que a corticofobia medida pela escala visual anal&#243;gica &#40;EVA&#41; estava associada a pior ades&#227;o&#59; entretanto&#44; ao aplicar o question&#225;rio TOPICOP na mesma amostra isso n&#227;o ocorreu&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">9</span></a> Ainda n&#227;o foi definido ponto de corte nos escores do TOPICOP que possa ser classificado como clinicamente relevante&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Frequentemente s&#227;o atendidos pacientes no Sul do Brasil que n&#227;o sabem que uma formula&#231;&#227;o espec&#237;fica cont&#233;m esteroides&#46; Portanto&#44; embora as taxas de corticofobia tenham sido compar&#225;veis &#224;quelas observadas em pa&#237;ses desenvolvidos&#44; &#233; poss&#237;vel que n&#227;o afetem no mesmo grau a ades&#227;o ao tratamento no Brasil por esse motivo&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">Diferentemente do presente estudo&#44; trabalhos anteriores descreveram associa&#231;&#227;o entre n&#237;vel educacional e corticofobia&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a> No entanto&#44; existe a possibilidade de que caracter&#237;sticas culturais possam estar envolvidas&#44; uma vez que a maioria dos estudos foi realizada em pa&#237;ses ocidentais do Hemisf&#233;rio Norte&#46; Um estudo chin&#234;s n&#227;o encontrou nenhuma associa&#231;&#227;o&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">8</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">O presente estudo sugere que as pessoas que obt&#234;m informa&#231;&#245;es sobre CST de amigos e familiares podem ter menos corticofobia&#44; o que est&#225; de acordo com Song et al&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> Isso &#233; especialmente relevante no Brasil&#44; onde os CST s&#227;o vendidos livremente em farm&#225;cias&#44; sem necessidade de receita m&#233;dica&#46; Mais da metade dos pacientes do presente estudo compraram CST sem receita m&#233;dica no passado&#46; Apenas 65&#37; responderam que o dermatologista era uma de suas principais fontes de informa&#231;&#227;o&#46; Isso indica a necessidade de educar melhor os pacientes ao prescrever tratamento para DA&#46; Curiosamente&#44; um recente estudo portugu&#234;s encontrou associa&#231;&#227;o inversa entre educa&#231;&#227;o em sa&#250;de e a corticofobia&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Parece n&#227;o haver associa&#231;&#227;o entre corticofobia e ades&#227;o ao tratamento no Sul do Brasil&#46; Al&#233;m disso&#44; n&#237;veis educacionais mais baixos est&#227;o associados ao maior grau de corticofobia entre cuidadores de crian&#231;as e adolescentes com DA e menor ades&#227;o entre adultos&#46;</p><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0005">Suporte financeiro</span><p id="par0085" class="elsevierStylePara elsevierViewall">Este projeto recebeu financiamento do Fundo de Incentivo &#224;
